Some people think that they are on the web they are relatively anonymous internet surfing.   I’m afraid nothing could be further from the truth, when you request a web page all  the information is passed to your web browser in clear text, your IP address is left in the web server logs and it would be relatively trivial to find out who and were you lived from that information from your ISP.

I’m afraid there’s more, your employer will have a full and exact list of all websites accessed, how long and when they were accessed.  I have sat in security meetings with huge charts displayed on the wall with the exact web sites that employees have accessed during the previous month and I can tell you there are some amazing sites!  Do these people know they can be traced I suspect not as they certainly wouldn’t be surfing those sort of sites at work if they did.

Anonymous Internet surfing by default simply doesn’t happen but it is possible.  The most basic way of protecting your identity is to use an anonymous proxy to hide your IP address when you are surfing.  Your communications can still be intercepted as the majority of that web traffic is in the clear but it’s less likely unless someone is deliberately targeting you, the main benefit is that your IP address is not left in the logs of the web servers and your web browsing will remain more private.

This is where many people make a really big mistake, this is what usually happens;

  1. User decides they want keep their surfing anonymous
  2. They search the internet for information and discover about anonymous proxies
  3. They look for a list of free anonymous proxies and find one near them
  4. They add the anonymous proxy settings to their internet browser and feel much more secure and private!

Unfortunately as is often true, anything that is free on the internet often comes with a hidden cost.  Our privacy seeking internet surfer is indeed now masking their IP address from web servers they visit, all the logs will store the IP address of the proxy server as that is what is actually visiting the web site.  The Proxy server is then passing back all the requests back to the user, all the users web traffic now goes through this proxy server.

Of course many hacking groups realise the value of proxy servers for privacy and it is why you’ll often find these lists prepared by hacking groups themselves then carefully allowed to drift out onto the internet by various means.   Our hapless surfer may come across one of the many lists which are riddled by hacked servers which have had proxy servers installed onto them (usually without the owners knowledge).   These servers specifically are monitored and data logged - you are in fact sending all your web traffic directly to a bunch of criminals or hackers, not such a great idea.

This really happens and more often than you think so please if you are going to use an anonymous proxy, make sure it is legitimate and the owner can be trusted.   Don’t just use a free one you found on an internet list - not only will you have a limited anonymous internet surfing capability, you could very well be sending all data to a bunch of Eastern European Identity Thieves.

Of course there are commercial and secure services that can secure your identity and protect your privacy.  I use a service which completely encrypts my who data stream with a US Military grade cipher (AES) and then uses a selection of high speed private proxies to make my browsing completely secure and anonymous and also safe from anyone who tries to eavesdrop.

Last year Google generated $18.12 billion in revenue while its chief pay per click rival Yahoo added $7.12 billion in revenue to its balance sheet. The majority of these revenues came from the popular and traditional pay per click advertising model. Much has been written about the possible merger between these conglomerates. What would a merger between these companies mean to you, the Search Engine Marketer?

Originally hailed as one of the best advertising creations ever, pay per click marketing has become the heart and soul of many traffic generation programs. Advocates contend that you are not paying for visibility as in traditional advertising but instead are targeting those interested individuals who “click” on your advertisement. The early adopters claimed that this made advertising accountable and allowed marketers to target their message and measure its response. What originally started out as a novelty in traffic generation has become an estimated $40 billion a year industry.

Microsoft, which currently has a market capitalization of $240 billion, has been a distant third place in the pay per click sector which has led many to speculate that its interest in Yahoo was primarily intended to make it more competitive with Google. As I write these words the news wires are reporting that Venture Capitalist Carl Icahn is in a proxy battle to deliver Yahoo to Microsoft. Meanwhile Yahoo is in talks with Time Warner while the US Department of Justice is doing an initial exploration and investigation about the legalities of Yahoo and Google merging. These are the “days of our clicks.”

Trust me, the pay per click soap opera is just warming up.

Throughout the intense drama and wild ups and downs of hearsay negotiations, I am all for a merger. Not because I’m a huge Yahoo, Google or Bill Gates fan, but because I believe the health of Internet is defined by the clear standards and practices of a sane search engine marketing marketplace. Currently, Search Engine Marketers have had to contend with “Google Slaps” where their keyword campaigns have been shut down by the Silicon Valley giant with no explanation provided.

PPC Marketers who have suffered the infamous Google slap compare it to Microsoft’s Blue Screen of Death in the PC arena. When Google “Slaps”, a pay per click marketer has no choice but to reboot their entire pay per click marketing campaign with no assurance of a different outcome in the future. Industry pundits debate whether a merger between Google and Yahoo would result in a “kinder and gentler” pay per click search engine.

Marketers online are interested primarily in four factors:

1) Price of a Click

2) Volume of Traffic Available per keyword

3) Number of Conversions Occurring Per Offer

4) Overall Quality of Traffic

However with all of the speculation surrounding the possibility of a merger between these companies, marketers now will have to evaluate the educational learning curves associated with understanding and mastering the distinctive delivery platforms, search marketing algorithms and placement possibilities of each company. I know that sounds like a mouthful but compare for a second how Google and Yahoo define quality as far as your pay per click marketing campaigns are concerned.

Twenty Four Months ago Google launched something they refer to as their “Quality Score” for marketers to determine the effectiveness of a keyword pay per click campaign. Google claims that a high quality score will result in lesser costs for your keyword terms. Apparently a high quality score is determined by having a high click through rate on a keyword, the quality of your landing page, your accounts historical performance and “other” relevant factors. Okay, sounds pretty straightforward, no problems thus far! But I must admit that “other” word sure is a doozy in the real world. (Must’ve been drafted by paralegals!) A few months later Yahoo paraphrased the adwords playbook and released their “quality index.”

The overriding concern most search engine marketers have is one of seeing too much power in one platform. When Google implemented its quality score guidelines, pay per click costs actually increased for affiliates whose livelihoods are completely dictated by their abilities to drive traffic cost effectively. My experience has shown that keyword costs on Google average 25% higher than on Yahoo or Microsoft. A merger of Yahoo and Microsoft would certainly offer a more cost effective alternative for search engine marketers who are focused on the price they pay for each click. The combined search market share of Yahoo and Microsoft is estimated at 31.5%. Meanwhile the search market share of Google by itself is 56.3%. A merger of the Yahoo and Microsoft platforms would certainly create a better distribution of “power” and lower cost per click for search engine marketers. Pay per click costs have been in a perpetual bull market for over ten years. Anything that slows down the price that I have to pay for a click or offers a potent alternative is something I will completely support.

The second major issue search engine marketers have arises from not understanding why identical keyword campaigns running on both Google and Yahoo often receive mixed “quality score” or “quality index” signals? My experience has been that these identical campaigns will often receive the blessing from one search company while simultaneously receiving the wrath of the other. I have asked pay per click experts as well as employees of Google and Yahoo to elaborate on this dilemma and, to be honest with you, understanding their responses is harder than Chinese Differential Calculus. I have more success playing with my Rubiks Cube in the dark!

How can the two leaders in the pay per click marketing industry hold arbitrary, mysterious standards in their sacred pay per click algorithms and be expected to play well together after a merger has occurred?

This issue begs the question that if Google and Yahoo do not see eye to eye with regards to defining “quality” as far as keyword campaigns are concerned what makes anyone think that a merger between these two giants is going to bode well for search engine marketers? Search Engine marketers are focused on cost, volume and conversions. That is how they define quality. And as far as I can tell that is a thousand times better of a definition than Google, Yahoo or MSN will ever come up with.

Many park owners feel that it is their duty as the owner to rule with an iron hand. They think that they can cure all of the park’s ills with rule after rule. At many parks, the rules section is longer than the lease itself.

But is the park any better off with “rules phobia”?

I have tested operating parks with extreme rules enforcement, and also with virtually no rules enforcement at all. And I think I have found the solution to successful park rules.

Think Subdivision

For inspiration on proper rules enforcement, look no further than the nearest residential subdivision. These rules are found in the city’s code enforcement manual, and are monitored and enforced by the code inspector. They must be important, since most of the city lives under these guidelines. And what do they focus on? Only a few, very important items such as 1) no non-running vehicles in yards 2) no big trash or debris in yards 3) grass mowed to a certain maximum height 4) reasonable maintenance of your property. Is there really any reason for a mobile home park to expect, and enforce, greater rules on their residents? I would argue that there is no possible way that you can expect a park resident to be more proactive on rules than someone who lives in a subdivision.

Be Reasonable

If a resident has a car up on blocks while he is fixing the radiator for a few days, you cannot count that as the same offense as someone who has a car up on blocks for ten years, waiting to sell it off part by part. When you fail to see the gray areas, but only black and white, you will begin to really offend residents - and rightfully so. You would never accept such treatment yourself. You need to allow room in every rules problem for extenuating circumstances. These are another reason not to micro-manage. You will get caught up in too many special cases if you go crazy over hundreds of rules.

Enforcing Rules Costs You Money

The more rules you write and enforce, the more money it costs you. How? Every time you spend your time, or your manager’s time in writing letters and following up and meeting with residents regarding rules violations, it costs you money. In addition, when you have to kick residents out to set examples that you won’t allow your rules to be pushed around, you cost yourself legal fees, filing fees, and opportunity cost of a lot income lost. Remember that the average park as a multiplier of 10 times the net income in valuation, so that resident you just kicked out for too many loud music violations cost you $200 x 12 x 10 = $24,000. Was it worth it?

Nothing Scares Residents Away More Than Overly Tight Rules

The number one reason I’ve found for a resident to come to our park and talk about moving their mobile home over, is ridiculously tight rules enforcement. To many residents, you create a “prison” atmosphere when your rules are too intrusive. Would you want to have that kind of rules enforcement by the code officer in your subdivision? Or would you go nuts and tell the wife and kids “that’s it - we’re out of here, I’m putting the home on the market” after you receive your fiftieth code violation ticket?

Conclusion

Rules are important. But they can be taken too far. Remember that it’s pretty crazy for you to expect more than a subdivision. And every minute spent on rules is costing you, not making you, money. So ease up, and let those residents alone. You will be richer and happier, and so will your resident base.

The last decade of the previous century witnessed accelerating proliferation of digital technology in health care, which, along with reduced costs and greater service quality, introduced new and greater risks for accidental disclosure of personal health information.

The Health insurance Portability and Accountability Act (HIPAA) was passed in 1996 by Congress to establish national standards for privacy and security of personal health data. The Privacy Rule, written by the US Department of Health and Human Services took effect on April 14, 2003.

Failure to comply with HIPAA risks accreditation and reputation damage, lawsuits by federal government, financial penalties, ranging from $100 to $250,000, and imprisonment, ranging from one year to ten years.

Protected Health Information (PHI)

The key term of HIPAA is Protected Health Information (PHI), which includes anything that can be used to identify an individual and any information shared with other health care providers or clearinghouses in any media (digital, verbal, recorded voice, faxed, printed, or written). Information that can be used to identify an individual includes:

  1. Name
  2. Dates (except year)
  3. Zip code of more than 3 digits, telephone and fax numbers, email
  4. Social security numbers
  5. Medical record numbers
  6. Health plan numbers
  7. License numbers
  8. Photographs

Information shared with other healthcare providers or clearinghouses

  1. Nursing and physician notes
  2. Billing and other treatment records

Principles of HIPAA

HIPAA intends to allow smooth flow of PHI for healthcare operations subject to patient’s consent but prohibit any flow of unauthorized PHI for any other purposes. Healthcare operations include treatment, payment, care quality assessment, competence review training, accreditation, insurance rating, auditing, and legal procedures.

HIPAA promotes fair information practices and requires those with access to PHI to safeguard it. Fair information practices means that a subject must be allowed

  1. Access to PHI,
  2. Correction for errors and completeness, and
  3. Knowledge of others who use PHI

Safeguarding of PHI means that the persons that hold PHI must

  1. Be accountable for own use and disclosure
  2. Have a legal recourse to combat violations

HIPAA Implementation Process

HIPAA implementation begins upon making assumptions about PHI disclosure threat model. The implementation includes both pre-emptive and retroactive controls and involves process, technology, and personnel aspects.

A threat model helps understanding the purpose of HIPAA implementation process. It includes assumptions about

  1. Threat nature (Accidental disclosure by insiders? Access for profit? ),
  2. Source of threat (outsider or insider?),
  3. Means of potential threat (break in, physical intrusion, computer hack, virus?),
  4. Specific kind of data at risk (patient identification, financials, medical?), and
  5. Scale (how many patient records threatened?).

HIPAA process must include clearly stated policy, educational materials and events, clear enforcement means, a schedule for testing of HIPAA compliance, and means for continued transparency about HIPAA compliance. Stated policy typically includes a statement of least privilege data access to complete the job, definition of PHI and incident monitoring and reporting procedures. Educational materials may include case studies, control questions, and a schedule of review seminars for personnel.

Technology Requirements for HIPAA Compliance

Technology implementation of HIPAA proceeds in stages from logical data definition to physical data center to network.

  1. To assure physical data center security, the manager must
    1. Lock data center
    2. Manage access list
    3. Track data center access with closed circuit TV cameras to monitor both internal and external building activities
    4. Protect access to data center with 24 x 7 onsite security
    5. Protect backup data
    6. Test recovery procedure
  2. For network security, the data center must have special facilities for
    1. Secure networking - firewall protection, encrypted data transfer only
    2. Network access monitoring and report auditing
  3. For data security, the manager must have
    1. Individual authentication - individual logins and passwords
    2. Role Based Access Control (see below)
    3. Audit trails - all access to all data fields tracked and recorded
    4. Data discipline - Limited ability to download data

Role Based Access Control (RBAC)

RBAC improves convenience and flexibility of systems management. Greater convenience helps reducing the errors of commission and omission in granting access privileges to users. Greater flexibility helps implement the policy of least privilege, where the users are granted only as much privileges as required for completing their job.

RBAC promotes economies of scale, because the frequency of changes of role definition for a single user is higher than the frequency of changes of role definitions across entire organization. Thus, to make a massive change of privileges for a large number of users with same set of privileges, the administrator only makes changes to the role definition.

Hierarchical RBAC further promotes economies of scale and reduces the likelihood of errors. It allows redefining roles by inheriting privileges assigned to roles in the higher hierarchical level.

RBAC is based on establishing a set of user profiles or roles according to responsibilities. Each role has a predefined set of privileges. The user acquires privileges by receiving membership in the role or assignment of a profile by the administrator.

Every time when the definition of the role changes along with the set of privileges that is required to complete the job associated with the role, the administrator needs only to redefine the privileges of the role. The privileges of all of the users that have this role get redefined automatically.

Similarly, if the role of a single user is changed, the only operation that needs to be performed is the reassignment of the user profile, which will redefine user’s access privileges automatically according to the new profile.

Summary

HIPAA compliance requires special practice management attention. A practice with multiple separate systems for scheduling, electronic medical records, and billing, requires multiple separate HIPAA management efforts. An integrated system reduces the complexity of HIPAA implementation. By outsourcing technology to a HIPAA-compliant vendor of vericle-like technology solution on an ASP or SaaS basis, HIPAA management overhead can be eliminated (see companion papers on ASP and SaaS for medical billing).
